[docbook] Annotations again

A couple of months ago, the TC revisited annotations. We discussed
some of the experiences that I had implementing annotations[1][2]. On the
basis of those discussions, I've changed a few things[3].

I'd like to think that we're beginning to reach a consensus position
on how annotations should be supported in DocBook.

Comments, etc., most welcome.
